About the Role
The Support Coordination Manager is responsible for the strategic leadership, operational performance, and growth of United Foundation’s national Support Coordination service. This role leads a national team delivering support coordination to NDIS participants and is accountable for service quality, workforce performance, referral growth, and financial sustainability.
The position also oversees the design and development of operational systems and reporting infrastructure, ensuring the organisation maintains a scalable and efficient service model supported by integrated technology platforms including Salesforce and Maica.
Working closely with the executive team, the National Manager contributes to organisational strategy, new service development, and sector partnerships, positioning the organisation as a leading provider of support coordination and housing pathways within the NDIS ecosystem.
Lead and manage the national Support Coordination program to ensure high quality, participant-centred service delivery.
Ensure services are delivered in accordance with NDIS Practice Standards and organisational policies.
Establish operational frameworks that support consistent service delivery across the national team.
Lead the growth of the Support Coordination program through the development of strong referral pipelines.
Oversee intake processes to ensure timely onboarding of new participants.
Develop initiatives that increase program visibility and referral pathways.
Lead the design and continuous improvement of operational systems supporting the Support Coordination program.
Develop reporting frameworks and dashboards to monitor service performance, utilisation and growth.
Contribute to organisational strategy through analysis of sector trends, participant needs and operational data.
Identify opportunities to enhance the organisation’s role within the disability and housing support ecosystem.
Support organisational growth through the integration of newly acquired support coordination teams.
About You
Your passion for exceptional service, particularly for those with physical and psychosocial disabilities and homelessness, will drive the success of our nationwide support coordination services.
Tertiary qualification in Social Work, Psychology, Community Services, or a related field.
Extensive experience within the disability or community services sector.
Demonstrated experience managing service teams and operational programs.
Strong understanding of the National Disability Insurance Scheme.
Experience driving service growth, operational improvement and sector partnerships.
Experience designing or implementing CRM systems or operational reporting frameworks is highly regarded.
